Comments:
An unannounced monitoring inspection was completed on 11/7/19. There were 78 children present in six groupings. Seven children's records and five staff records were reviewed. There were no medications at the center.

The inspector arrived at the center at 8:30 am and departed at 1:15 pm.

The children were observed during free times in the classrooms, during outside time, during structured activities in the classrooms, during lunch, and during nap. All infant activities were observed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all required information was documented in each child's record.

Evidence:
1. There were no work phone numbers for the mothers of children #6 and #7. The children's records had a place of employment documented for both mothers.

2. There was no work phone number documented for the father of child #1. There was a work place documented for the father.

Plan of Correction: This was corrected during the inspection. The site coordinator obtained and documented the work phone numbers for all of the parents.

Standard #: 22VAC40-185-370-5
Description: Based on observation, the center failed to ensure that awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs. The center also failed to ensure that infants were protected from older children.

Evidence:
1. There was an infant, eight weeks old, observed to be awake and secured in an infant bouncy seat at arrival of the inspector into the infant room at approximately 10:45 am. The infant was observed awake at 11:22 am and still secured in the same bouncy seat. The infant was not provided a change in play space in over 30 minutes and was awake for over 30 minutes in the same play space.

2. A nine month old infant was observed to be asleep in a bouncy seat. A 13 month old child was observed to hit the infant in the head twice in less than two minutes with a plastic ring toy with multiple attached manipulative plastic toys on this ring.

Plan of Correction: The child was removed from the bouncy seat at approximately 11:25 am. A third staff person will assist with preparing lunch and as needed so the infants are protected from the older children.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that diapering was practiced as required.

Evidence:
1. A diaper change was observed of an infant. The staff person changing the diaper was observed to dispose of the soiled diaper and soiled wipes in the trash can using one hand to open the lid. The diaper and wipes were soiled with a loose bowel movement. The trash can used was a foot operated can but was turned in such a way that the foot pedal was not accessible to staff. The foot pedal was turned to the side where the wall and another piece of equipment was blocking the foot pedal. The Standards require That diapers be disposed of in in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: When the licensing inspector reviewed this with the staff person, the trash can was turned so the foot pedal could be accessed during diapering.
