Comments:
A self reported incident was reported by Culpeper Baptist Child Development Center, Inc. on 11/4/19. An inspection was conducted regarding the subsidy health and safety requirement to reporting serious injuries within two days. A violation was found as a result of the inspection and can be found on the violation notice.

Violations:
Standard #: 22VAC40-665-550-2
Description: Based on interview and a review of injury reports, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence:

1. On 11/4/19 the vendor contacted the licensing inspector to advise two injuries occurred at the center that resulted in children needing medical attention from a professional. Neither incident was reported within two days.
2. On 11/4/19 an injury report was received with the incident dated 10/11/19 and a second injury report was received on 11/4/19 with the incident dated 10/24/19. Both documented the children were treated by a medical professional as a result of the injuries.

Plan of Correction: In the future injuries to a child while under the center's supervision and a referral is made for a treatment from a medical professional will be reported to licensing no longer then two days after the date of the incident.
