An investigation was initiated on 9/4/19 in response to a complaint received by the Eastern Regional Office on that same date regarding concerns in the areas of physical plant, infant feeding and nutrition and food services. An unannounced on-site inspection was conducted on 10/17/19. Interviews with staff were conducted and a tour of the center, including all classrooms, was completed. Based on information gathered during the investigation, the portion of the complaint regarding physical plant is determined to be valid. Violations cited are identified on the violation notice and were discussed with the center owner on 11/1/19.

Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. There are multiple areas of peeling paint on the walls in the toddler classroom and school-age classroom.
2. The power cord to the window air conditioning unit in the toddler classroom was loosely coiled and dangling from the unit at child height, which poses a strangulation hazard.
3. There is a wooden play kitchen set in the pre-k classroom with a broken door and broken metal hinge, creating a potential pinch point.Plan of Correction: The center responded with the following: Broken kitchen set was removed during inspection on 10/17/19. The window unit in the toddler classroom was removed following the inspection on 10/17/19. Painting and other minor repairs in the building has been scheduled and work will begin 11/9/19.
Standard #: 22VAC40-185-500-B Complaint related: No Description: Based on observation it was determined the center did not ensure that the diapering surface was nonabsorbent.
Evidence:
1. The diaper changing pad in the waddler classroom has multiple tears and is therefore no longer nonabsorbent.
2. The center owner confirmed that the diaper changing pad in the waddler classroom is torn.Plan of Correction: The center responded with the following: The replacement for the torn changing pad in the waddler classroom had already been ordered at the time of inspection. It was received and replaced on 10/18/19.
