63.2 Child Abuse & Neglect

Technical Assistance:
Discussion was held on the topics of injury reports and injury prevention plan.

Comments:
An unannounced monitoring inspection was conducted on 11/06/2019 from approximately 2:50 pm through 5:20 pm. On this date 64 children were in care with 5 staff members present. Children were observed during arrival, washing hands, snack time, "Homework Club" and transitioning to outdoor play time. Children were engaged coloring, playing with blocks, puzzles and other manipulatives. The site was clean, organized and contained an varied supply of games and materials. Staff were observed taking attendance, supervising children during homework, free play and transitions to and from the bathroom and playground. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, injury reports, required posted documents, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-40-I
Description: Based on document review, the center did not develop written procedures for injury prevention based on documentation of injuries.
Evidence:
1) On the date of inspection, an injury prevention plan was not available for review.

Plan of Correction: We will make a summary of the injuries and complete the report.

Standard #: 22VAC40-185-60-A
Description: Based on record review, not all children's records contain all required information.
Evidence:
1) The record for Child #1 did not contain a work place for two parents and work phone for one parent.
2) The record for Child #3 did not contain a work phone for one parent.
3) The records for Child #4 and Child #5 did not contain addresses for two emergency contacts.

Plan of Correction: I will have parents complete the information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, not all staff records contained all required information.
Evidence:
1) The records for Staff #2 and Staff #3 did not contain an address for an emergency contact.
2) The record for Staff #4 and Staff #5 did not contain documentation of 2 or more references being verified.
3) The record for Staff #1 did not contain documentation of name, address, verification of age requirement, job title, and date of employment; and name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: Some information was collected during inspection. I will ask the staff to complete the missing information .

Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, not every staff supervising children completed mandatory training by the end of their first day of assuming job responsibilities as required .
Evidence:
1) The record for Staff #1 (start date unknown), Staff #2 (start date 09/19/2019), Staff #4 (start date 12/11/2018) and Staff #5 (start date 05/09/09) did not contain documentation of having completed the training for recognizing child abuse and neglect.
2) The record for Staff #1 (start date unknown) did not contain documentation of having completed the orientation training.

Plan of Correction: We will schedule for the staff to complete the training or provide the documentation.

Standard #: 22VAC40-185-550-D
Description: Based on document review, The center did not practice monthly evacuation drills as required.
Evidence:
1) The evacuation drill log from September 2018 through October 2019 did not contain drills documented for the months of December 2018 and February 2019.

Plan of Correction: We will make sure to record monthly evacuation drills.
