Comments:
An unannounced monitoring inspection was conducted today from 3:05pm - 5:00pm. There were 80 children in care ages five through eight years old with five teachers at the onset of inspection. Children records and staff emergency contact information were reviewed on site. Staff records are housed at a central location and will be reviewed at a later date. A walking of the cafeteria gym, outside play space and rest rooms was completed. Required postings, annual inspections, emergency evacuation drills first aid kit and medication were reviewed. Children were observed in teacher and self-directed activities including restroom routines, hand washing, snack, homework and learning center play. Violations regarding staffing and supervision and programs were identified appear on the violation notice. The violations were discussed with the program director on site at the conclusion of the inspection.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and interview the center failed to ensure that for children from age of eligibility to attend public school through eight years, one staff member for every 18 children.
Evidence:
1 - Today from approximately 3pm through 3:48pm in one of three groupings of children, ranging in age from seven through eight years old there were 39 children in care with staff 1 and staff 2 in the gym. There were not any other staff in the room.
2 - Staff 1 stated that since the beginning of the 2019 - 2020 school year the program has been out of ratio on a regular basis, specifically on Tuesdays and Thursdays.

Description: Based on observation and interview the center failed to make provision for an individual place for each child's personal belongings.
Evidence:
1 - 30 backpacks were piled on the ground and on top of each other, on the playground while the kindergarten class was outside.
2 - Staff 4 confirmed that there was not an individual place provided for each child's personal belongings.

Description: Based on observation and interview the center failed to practice two shelter-in-place drills per year for the most likely to occur scenarios.
Evidence:
1 - Log sheets maintained of the center emergency evacuation drills shows documentation for only one shelter-in-place drill during the 2018 - 2019 year.
2 - Staff 1 confirmed that there was documentation of only one shelter-in-place drill during the the 2018-2019 year.
