Documentation of children's injuries was discussed with the program director.
- Comments:

An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 10:40 AM and departed at 2:00 PM. Morning activities, outdoor play and lunch service were observed. A sample size of ten staff records and ten children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-550-M Description: Based upon review of injury records and staff interview, the facility has not ensured that the record of injuries includes the staff present at the time of the injury.
Evidence:
1. An injury record dated 5/21/2019, for child 11 did not include the names of staff present at the time of the injury.
2. Staff 1 verified that the injury report for child 11 did not have indicated the staff present.Plan of Correction: The facility responded with the following:
All staff will receive additional training regarding the importance of completing all required information on the injury records.
