Technical Assistance:
Technical assistance today included record keeping, physical environment and medications.

Comments:
An unannounced renewal inspection took place today from 10:45 am - 2:30 pm. There were 64 children present with 10 staff. I observed children building with Legos and blocks, singing ABCs, involved in a nature craft project beginning lunch and naptime. Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some areas of the center that were not maintained in safe condition.
Evidence:
There were stacked metal chairs accessible near children in two rooms on the upper level of the school.At front door, the mat is beginning to tear. and pose a tripping hazard.

Plan of Correction: Chairs will be removed and the mat will be replaced.

Description: Based on observation during today's inspection, there were some hazardous substance not locked as required.
Evidence:
There were some cleaners and spray cans on a shelf in one classroom during naptime..

Description: Based on observation during today's inspection, there were some outlets not covered on an extension cord located in the cafeteria.

Description: Based on a observation during today's inspection, there was no posting at the center regarding a communicable disease.
Evidence:
Two children went home today and staff reported a flu-like virus which has been impacting the center, but there was no posted communication today regarding the symptoms of the illness.

Description: Based on a review of children's medications today, there was a medication present that was no longer authorized.
Evidence:
The medication for Child # 4 had an expiration date of 10/23/19, but was still at the present at school.

Plan of Correction: We gave the parent a new form to fill out.

Standard #: 22VAC40-185-550-M
Description: Based on a review of incident reports, not all information was included on reports.
Evidence:
Of five reports reviewed, the incident reports on 11/1 and 10/30 did not include a response to "future action to prevent further injury"

Plan of Correction: We will re-train the staff to complete the forms.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records today, there were some results of the the Search of the Registry for VIrginia and other states that did not have results of the searches documented.
Staff # 2, who began work on 8/19/19, did not have the results of the Search of the Central Registry available; and Staff # 3, who began work 9/30/19, did not have results of the Virginia Search of the Registry or for the state where she resided in the last five years.

Plan of Correction: We contacted the DSS Background office today and found out that checks were received but not processed yet, and the one for North Carolina was sent today.There was a question with a misspelling of the name of the center.
