Comments:
An unannounced renewal inspection was conducted on Monday, November 4, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:05 am and concluded the inspection at approximately 2:20 pm. The center's director and assistant director were present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 142 children in the direct care of 21 staff members. Additional staff were present and available to the children in care. Upon the inspector's arrival, the children were engaged in a variety of activities. In the infant classrooms, some staff were observed sitting on the floor with a group of infants providing individual care and gross movement activities. Other staff were rocking and feeding children. Diapering and hand-washing procedures were also observed in these rooms. The children in the toddler classroom were observed preparing for lunch. They were singing with staff, taking bathroom breaks, and washing hands. Today?s lunch consisted of macaroni and cheese, green beans, raspberries, and milk. Additional observations in other preschool classrooms included the children playing with blocks, legos, and puzzles. The classrooms had time outside on the playground as well. The center was decorated with children's art and seasonal decorations. All areas of the facility were inspected. The facility has age appropriate materials and supplies. Staff interactions with children were positive. The following information was posted: license, menu, daily schedule, emergency numbers, evacuation procedures and various parent information. The inspector interacted with staff and children in care.

Ten children's records, four medications, and ten staff records were reviewed. The first aid kit, flashlight, and weather band ratio were observed and in compliance. The most recent evacuation drill was documented on 10/31/2019. The most recent shelter-in-place drill was documented on 10/01/2019.

Please specify how the deficient practice will be or has been corrected. Standard #: 22VAC40-185-60-A
Description: Based on a review of ten children's records and interview on 11/04/2019, the center did not ensure that one child's record contained the required information.

Evidence: 1) The record for Child #10, enrolled on 09/03/2019, was missing the address of two designated people to call in an emergency if a parent cannot be reached and the work phone number and place of employment of each parent who has custody. 2) During interview, a member of management reported the missing information in the record for Child #10 could not be located at the time of the inspection.

Plan of Correction: Per the Center: "All necessary information was collected and added to the file on 11.05.2019. All student files are currently under a full review by our Administrative Coordinator. This review will then be audited by our Assistant Director. School Administration will use the enrollment, monthly, and annual student file audit procedures to ensure ongoing compliance to this standard."
