Comments:
A mandated Monitoring inspection was conducted today from 10:40am to 1:00pm. There were 69 children present with 13 staff. Children were observed learning to use scissors, playing in sensory tables, reading books, playing "car repair shop",practicing letters, learning sequencing, having snack (goldfish, cheese corn chips, apple sauce, popcorn and string cheese) washing hands, and being dismissed for the day. Reviewed 2 children's files, 3 staff files, attendance, emergency evacuation drill log, medications and authorizations, first aid kits and physical plant. Standard #: 22VAC40-185-60-A
Description: Based on record review, each child's record did not contain documentation that the child's proof of identity and age had been viewed
Evidence:
The center did not document they had viewed Child#1 proof of identity and age.

Plan of Correction: We will obtain the information from the parent and document.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous items such as cleaning agents were not kept in a locked place.
Evidence:
Clorox wipes were observed on an open shelf in the Teddy Bear room.

Plan of Correction: We will remind staff that they need to be behind a locked door.
