Comments:
An unannounced monitoring inspection was conducted on Monday, October 21, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 4:00 PM and concluded the inspection at 6:00 PM. The census for today's inspection consisted of 86 children. During the inspection, children were playing in the gym, on the playground, completing homework and having snack. Children's equipment, learning materials, indoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed.

Medication is administered and medication, authorization forms and allergy plans were reviewed. Nine children's records and nine staff emergency contact records, fire drill and shelter in place drill logs, daily attendance, and all required postings were reviewed.

The administrator and another member of management were available for the inspection and were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center did not ensure each child obtained documentation of a physical examination by or under the direction of a physician prior to attendance or within one month after attendance.

Evidence:
1. The record for child #1 (date of enrollment 8/6/2019) and child #2 (date of enrollment 9/3/2018) did not contain documentation of a physical examination.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the facility did not ensure that each record contains all required information.

Evidence:
The records for child #3 (date of enrollment 9/27/2019), child #5 (date of enrollment 9/03/19) and child #6 (date of enrollment 9/03/19) did not have documentation of viewing proof of the child's identity and age.

Description: Based on an interview, the center did not ensure that the required information for each staff was kept at the center.

Evidence:
1. The center did not have documentation of the name, address and telephone number of a person to be notified in an emergency for staff #3, #4 and #5.
2. The center did not have documentation of any health problems which may interfere with fulfilling the job responsibilities for staff #3, #4 and #5.
