63.2(17) License & Registration Procedures

Comments:
An unannounced non-mandated monitoring inspection, a requirement of the Conditional License, was completed on 11/12/2019. There were 17 children, ages 7 months-2 years, and 7 staff present during the inspection. The inspector reviewed 5 children's record, 5 staff records and 1 medication during the inspection. Children were observed in the following activities: having lunch, during nap time and in free choice activities. Infants were observed being fed and allowed to sleep on demand. Proper diapering, hand washing and table sanitizing were also observed. The inspector reviewed the emergency supplies during the inspection. The inspector discussed subsidy, infants and nutrition with the staff. The inspector arrived for the inspection at 10:30 a.m. and departed at 1:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-270-C
Description: Based on measurement of temperature, the facility failed to ensure that in inside areas occupied by children, the temperature shall be maintained no lower than 68 degrees Fahrenheit.

Evidence: The temperature inside of the facility measured 63.7 degrees Fahrenheit at 12:45 p.m., 63.9 degrees Fahrenheit at 12:55 p.m. and 64.2 degrees Fahrenheit at 1:00 p.m. following the heat in the building running a full cycle.

Plan of Correction: The building owners will be contacted to look at the heat.

Standard #: 22VAC40-185-290-3
Description: Based on observations and discussion with the staff, the facility failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers.

Evidence: There was an uncovered electrical outlet in the Infant Room. The outlet was accessible to children playing on the floor.

Plan of Correction: The outlet will be covered.

Standard #: 22VAC40-185-370-1
Description: Based on observations and discussion with the staff, the facility failed to ensure that when an infant is placed in his/her crib, he/she shall be placed on his/her back (supine).

Evidence: Staff #2 was observed placing Child #6, determined to be 12 months old, on their bully (prone) into a crib for sleeping.

Plan of Correction: Staff will be instructed to place all children up to 16 months on their backs when sleeping.

Standard #: 22VAC40-185-500-B
Description: Based on observations, the facility failed to ensure that the diapering area shall be a nonabsorbant surface.

Evidence: The changing table pad closest to the restroom had 2 tears in it exposing the nonabsorbant surface of the changing pad.

Plan of Correction: Will buy a new pad.

Description: Based on record review and discussion with the staff, the facility failed to ensure that a person must not be employed until the agency has the person's completed sworn statement or affirmation.

Evidence: The record for Staff #2 had a documented hire date of 10/24/2019; the date on the sworn statement is 10/25/2019.

Plan of Correction: All staff will sign sworn statement before hire date.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and discussion with the staff, the facility failed to obtain a central registry finding within 30 days of employment.

Evidence: The record for Staff #4 has a documented hire date of 09/12/2019; there were no central registry finding in the record for Staff #4 during the inspection.

Plan of Correction: In the future follow-up will be documented and continual follow-up will take place.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and discussion with the staff, the facility failed to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.

Evidence:
1. The record for Staff #3 had documentation of having resided outside of the state of Virginia within the preceding five years; there was no record of having requested a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by the state in which the individual resided in the preceding five years for any founded complaint of child abuse or neglect against him. The documented hire date for Staff #3 was 09/10/2019.
2. The record for Staff #4 had documentation of having resided outside of the state of Virginia within the preceding five years; there was no record of having requested a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by the state in which the individual resided in the preceding five years for any founded complaint of child abuse or neglect against him. The documented hire date for Staff #4 was 09/12/2019.

Plan of Correction: They will be requested.
