Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, medication administration forms, children and staff records. Administrator stated several forms were used for children enrollment information and the center will have a consistent form that will meet the standard requirements .

Comments:
An unannounced monitoring inspection was conducted today from approximately 10:10 am-2:30 pm. There were 98 children supervised by 17 direct care staff. The physical plant, 7 staff records, 9 children?s records, 5 medications records, evacuation drills, injury reports, emergency supplies, and policies were inspected. The children were observed center and group activities, coloring at table tops, infants were sleeping in the crib room, engaging freely, story time, outside playgroup play. Diapering and hand washing procedures were observed. Staff were observed to be engaged with children throughout the inspection.

The playground and surfacing was inspected. There were a variety of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-130-A
Description: Based on children's record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #3 (start date 6/10/2019) did not have documentation of immunizations before attending the center available for review.

Plan of Correction: Immunization record has been submitted and placed in file.

Standard #: 22VAC40-185-130-B
Description: Based on children's record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #5 (birth date 1/02/2018) record did not have documentation of additional immunizations once every six months.

Plan of Correction: Parents notified of this standard. Managers will follow-up with parents to ensure we remain in compliance.

Standard #: 22VAC40-185-140-A
Description: Based on records review, 2 out 9 children's record ,the center did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child #3 (start date 6/10/2019) and Child #5 (start date 3/27/2018) did not have documentation of physical within 30 days available for review.

Plan of Correction: Information has been requested for parents. Managers will follow-up with parents to ensure we remain in compliance.

Standard #: 22VAC40-185-60-A
Description: Based on children's record review, 8 out of 9 records, the center did not maintain record requirements for enrolled children.

Evidence:
1. Child #1 's record were missing work phone number for parent.
2. Child #3 and #5 were missing one emergency contact person to call in an emergency if a parent cannot be reached.
3. Child #1, #4, #5, #6 were missing documentation of child updates and confirmation of up-to-date information in the child's record.

Plan of Correction: Most information needed was listed in other paperwork in file. For ease of review, managers will ensure that all information is duplicated throughout paperwork
