Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on October 3, 2019 regarding supervision. The inspection was initiated on November 4, 2019 and completed on November 4, 2019. On November 4, 2019, the inspector was on site from 12:00pm to 1:20pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff records. See the violation notice for more details.

Description: Based on record review, the center did not ensure that one staff record had all of the required information.
Evidence:
The record of staff #1 (start date: 11/8/17) did not have written information to demonstrate that the individual possesses the education required by the job position. The center's representative stated that staff #1 is a program leader.

Plan of Correction: Step 1- Staff #1 will provide a copy of their diploma before beginning work on 11/5/2019.
Step 2- When staff complete an educational program it will be required that staff submit completion documentation. This will ensure each staff possesses the education required by the job position.
Step 3- The director will ensure that staff #1 submits required education documentation on 11/5/2019. The director will also review all staff files monthly to ensure all education documentation is submitted when completed and/or before staff is promoted to program leader.
