Comments:
A Subsidy Health and Safety inspection was conducted on 11/15/2019 to review supplemental health and safety requirements.
An unannounced monitoring inspection was conducted today from 9:30am-1:30pm. There were 39 children (infants-4yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, being bottle fed, napping, coloring, playing pretend in the housekeeping, area and eating lunch. Today's lunch consisted of Sun-Butter, cheese, whole-grain flour tortilla, spinach, carrots, apple slices, and milk. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review and interview, the center did not obtain documentation for a staff member that the individual was evaluated by a health professional and is determined to be free of communicable tuberculosis (TB) prior to coming into contact with children.

Evidence:
Staff#4's record contains documentation of a tuberculosis screening/test that was obtained on 11/05/2019, the staff member's first day of employment was documented as 10/14/2019.

Plan of Correction: We will ensure that all the teachers have it prior to coming into contact with children, both TB test have been obtained and are in the file.
