Comments:
An investigation occurred on 11/07/2019 in response to a self reported incident regarding supervision received on 10/02/2019. Based on staff interview, children and staff record reviews, and observation, the preponderance of evidence supports a violation and the area of non-compliance is identified on the violation notice. Standard #: 22VAC40-185-340-F
Description: Based on staff interview, a child under 10 years of age was not always within actual sight and sound supervision of staff.

Evidence:
During transition from the toddler 2 room to the Movement Matter Zone room, Child #1 and Child #2 left the line and ran down the hallway towards the right. Staff #1 followed the two children to return them to the group while Staff #2 remained with the other children. At this time, Child #3 left the line, ran to the left and around the corner to a separate hallway. Staff #3 located in a classroom saw Child #3 in the hallway without supervision. Staff #3 notified Administration who returned Child #3 to the group. At this time, Staff #1 and Staff #2 became aware that Child #3 was out of sight and sound supervision. Staff stated Child #3 was without supervision for approximately 1 minutes.

Plan of Correction: Staff will always use the rope during transitions with one staff at the front and one at the back.
