Comments:
An unannounced monitoring inspection was conducted between the hours of 12:00 pm- 5:45pm. 72 children were in care with 16 staff members present. Children were observed during nap time, story time, singing and engaging in free play. Diapering, handwashing, and bottle feeding were also observed. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation that one child has received the immunizations required by the State Board of Health before the child's first day of attendance.

Evidence:
1. The first date of attendance for Child #2 is 7/31/17. The initial documentation of immunizations in the record for Child #2 was dated 10/20/17.

Plan of Correction: Immunizations were obtained for the child and placed in the file.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical within one month after attendance for each child.

Evidence:
1. The first date of attendance for Child #1 is 2/25/19. The physical exam in the record for Child #1 was dated 9/18/19.
2. The first date of attendance for Child #2 is 7/31/17. The physical exam in the record for Child #2 was dated 10/20/17.

Plan of Correction: Physicals were obtained for each child and placed in the file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment inside the center was not maintained in a safe and operable condition.

Evidence:
A rug located in the K-Prep room was observed to have the outer lining separated from the main rug causing a small loop, which created a potential tripping hazard for children

Plan of Correction: Corrected during inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, there is no documentation that a Child Abuse and Neglect check was obtained for one employee that has resided outside of the state of Virginia within the last 5 years.

Evidence:
Staff #1 (date of hire 11/5/13) has resided outside the state of Virginia within the last 5 years. The record for Staff #1 did not have documentation that out of state child abuse and neglect results were obtained. A child abuse and neglect form was submitted on 11/9/18, however there is no documentation of a follow-up with the agency regarding results.

Plan of Correction: I will call the agency and follow up.
