An unannounced monitoring inspection was conducted between the hours of 9:50 am-1:00pm. At the time of entrance, 60 children were in care with 10 staff members present. Children were observed during snack time, painting, identifying numbers, and playing independently. A selection of 4 staff and 5 children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, and the Emergency Preparedness Plan were reviewed. Evidence:
The most recent tuberculosis screening in the record for Staff #3 is dated 8/21/17.Plan of Correction: Staff will submit record of tuberculosis screening.

1. Staff #1 began employment on 7/01/19. The child abuse and neglect finding for Staff #1 was dated 10/15/19. Staff stated the child abuse and neglect check was sent out late.
2. Staff #2 began employment on 8/19/19. The record for staff #2 does not contain a child abuse and neglect finding.Plan of Correction: We will follow up regarding results.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, the center did not ensure that 1 of 4 staff submit to fingerprinting prior to first day of employment.
Evidence:
Staff #1 began employment on 7/01/19. The record for Staff #1 does not currently contain documentation of fingerprint results.Plan of Correction: Fingerprints were completed. I will request a copy from the Diocese.

Staff #1 began employment on 7/01/19. Staff #1 lived outside of the state of Virginia in the past 5 years. There is no documentation of a child abuse and neglect search for the state listed.Plan of Correction: The out of state search was completed. I will request a copy from the Diocese.
