Provided training resource for behavior guidance. Discussed the need to document the trainer's name when providing staff training.

An unannounced self reported incident was investigated on 10/21/19. A follow up unannounced inspection was completed on 11/15/19. A mandated inspection was also completed today.. A total of 71 children and 17 staff were on site today. Walk thru of all 9 classrooms was completed. Today was pajama/pizza day. The children were observed dressed in pajamas and preparing for lunch. Hand washing procedures and bathroom procedures were observed in the classrooms The book fair was also being held. The center was decorated with painted pumpkins, fall decorations completed by the children and a variety of artwork also completed by the children. The children transitioned to nap time. Staff were observed sitting beside the children on their cots encouraging sleep. The transitions from one activity to another allowed the children independence with their familiarity of the daily routine. Staff interviews conducted in the classrooms. Staff familiar with individual needs such as food allergies, medication needs and emergency procedures. A total of 5 staff records were reviewed between both inspections and 6 children's records. Standard #: 22VAC40-185-270-A Description: Based on observation of the classrooms, one area was not maintained in a clean condition.
Evidence: The diaper surface area in the Kitten Classroom was found dirty.Plan of Correction: We will clean this area immediately.
Standard #: 22VAC40-185-560-G Description: Based on observation of the Kitten class, food brought from home was not properly labeled.
Evidence: Yogurt in the refrigerator in the Kittens Class was not labeled with a child's name and was not dated.Plan of Correction: The yogurt was labeled immediately.
Standard #: 22VAC40-185-570-D Description: Based on a self reported incident, one infant was not fed according to parent instructions.
Evidence: Child #1's parent instructed staff to feed every two hours. On 10/10/19, staff failed to feed Child #1 at one 2 hour feeding.Plan of Correction: A new system of documentation of each bottle feeding was developed. Child #1 was napping during the scheduled feeding.
