Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-210 (Program leader qualifications); 22VAC40-185-(4)-270 (Building maintenance); 22VAC40-185-(4)-330 (Play areas)

Comments:
An unannounced monitoring inspection was conducted on 11/15/19 from 9:25am - 11:10am. During the inspection there were 45 children ages eleven months old through five years old in care with nine staff. Children were observed participating in various activities in the classrooms during the inspection. Records were reviewed for five children and five staff during the inspection. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. The record for child #5, present during the inspection, did not contain documentation of an immunization record.
2. Staff #6 reviewed the record for child #5, and confirmed there was no immunization record available for viewing during the inspection.

Plan of Correction: The facility responded:The parents of child #5 will be asked to provide a current immunization record. All new students will have a current immunization record prior to starting.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition.

Evidence:
1. The fence for the playground has two sections where the top rail has become detached.
2. In the Pre-K classroom, there is an outlet that has separated from the wall.
3. Staff #6 confirmed the items listed above were not in a safe and operable condition.

Plan of Correction: The facility responded: We will have the items listed above repaired.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview, it was determined that the licensee did not ensure that a nonabsorbent surface is used for diapering.

Evidence:
1. The diaper changing pad in the Toddler classroom had a tear approximately two inches long.
2. Staff #6 confirmed that the diaper changing surface was not nonabsorbent due to the tear in the changing pad.

Plan of Correction: The facility responded: We will purchase a new diaper changing pad and remind staff to inform center management whenever a tear develops in any of the diaper changing pads.
