An unannounced monitoring inspection was made on 11/7/19 related to a self-reported incident of forbidden actions that occurred on 10/9/19 and was reported to the inspector on 10/14/19. Video of the incident was reviewed and staff were interviewed.

Standard #: 22VAC40-185-410-1 Description: Based on review of video recordings and interview with staff, the center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child was not used as a form of punishment.
Evidence:
1. A video was reviewed of a 21 month old child being roughly handled by staff #1 on 10/9/19. On this date, staff #1 roughly placed the child to the floor on the child's back from an upright/standing position. The child was observed in the video, twice, to be dragged on the floor by one arm a short distance (approximately one to two feet) each of these times. According to staff #1 and staff #2 this rough handling of the child was done as a part of disciplinary action in response to the child biting another child in the classroom.Plan of Correction: Staff #1 was terminated when the administration was made aware of the incident.
