Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment. This is a requirement for subsidy providers.

Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on November 15, 2019 to review supplemental health and safety requirements. A licensing inspection was also conducted on this date (see separate report for details). Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection. There were 16 children present ranging in age from 2 months to 4 years. All staff must have documentation of the mandatory VA Pre-service Training and current certification in first aid and CPR within the first 90 days of employment. First aid and CPR certification needs to be renewed when applicable. When there are children with diagnosed food allergies enrolled, they must have an Allergy Action Plan completed by the physician. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. The last lockdown drill was conducted 9/9/2019.
