Technical Assistance:
A supplemental page was provided. As we discussed, your program will be using two main classrooms with two additional rooms to be used as needed. At this time no transportation will be provided by the center. All prescription and over-the-counter medications will be administered.

Comments:
Thank you for your time and assistance during the announced initial licensing inspection conducted on November 13, 2019. The on-site inspection was from 9:30 a.m. to 11:40 a.m. The facility and the outdoor play area were toured on this date. Measurements were taken for four classrooms. At the time of the inspection, no children were in care. The owner's record, one record for an officer of the Limited Liability Company, and one staff member's record were reviewed. Policies and procedures were reviewed for compliance, along with applicable licensing standards. Some items observed, reviewed, and discussed during the visit included: classroom set-up and arrangement, classroom equipment, first-aid kit, emergency supplies, napping equipment and linen care, cribs, emergency preparedness policy and procedures, the outdoor play space, required posted information, and water temperature. Compliance could not be determined for certain standards, as children were not in care. The exception to compliance was noted on the violation notice.

A recommendation will be made for the issuance of a Conditional Child Day Center license with a capacity of 25 children and an age range of six weeks through three years.

Standard #: 22VAC40-185-540-C
Description: Based on review of the first aid kit and discussion with the provider, the first aid kit did not contain all of the required items.

Evidence:
1. Missing from the first aid kit were: two triangular bandages, scissors, and a first aid manual.
2. The provider confirmed the items were not present.

Plan of Correction: The provider will obtain the required items today.
