An unannounced Monitoring Inspection was conducted today from 11:15am to 12:24pm. There were 37 (3-4 years) directly supervised by 7 staff. The physical plant, 2 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories.There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-160-A Description: Based on review, the facility did not ensure that each staff member submits documentation of a negative tuberculosis screening no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing.
Evidence: Reviewed 2 staff records and found that Staff 2 did not have documentation of a TB screening and has been employed for more than 21 days.Plan of Correction: Will obtain by the end of the month.
