Comments:
An unannounced monitoring inspection was conducted today. All rooms are in ratio during the inspection. 5 staff and 5 children's records were reviewed. All were in compliance. Children were observed resting quietly and playing with a variety of toys and games. First aid supplies, emergency supplies, medication authorizations, topical ointment authorizations, labeled bottles, and topical ointment labels are all in compliance. Violations:
Standard #: 22VAC40-185-420-E-1
Description: Based on a review of available documentation and staff statement, time infants spend on their stomachs is not being documented daily.
Evidence:
4 of 4 infants requiring 30 minutes tummy time daily, do not have the amount of time spent on stomachs documented on their daily sheet. No time is being documented in the app.

Plan of Correction: We will cover this topic on Saturday at our training. Administration will monitor the staff to ensure it is being documented

Standard #: 22VAC40-185-510-J
Description: Based on a review of medications, not all medications are stored locked,
Evidence:
6 of 7 medications reviewed were not stored locked. 5 medications were stored in Safety Sacks that did not closed/lock. 1 medication was stored in a ziplock bag. Medications were in a larger backpack hung in rooms.

Plan of Correction: New bags will be ordered. Medications will be separated to not over stuff the bags.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation, a monthly emergency evacuation was not conducted in July 2019.

Plan of Correction: Two emergency evacuation drills will be conducted in December.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury reports, all required components are not documented.
Evidence:
5 of 11 injury reports reviewed did not contain documentation of the time of parent notification.

Plan of Correction: We will review the injury form at our training on Saturday. All staff will be retrained on how to complete the form.
