Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 11/20/2019. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Recommended use of the VDSS medication administration form for clarity. Currently the center is utilizing several different forms. Authorizations from other entities (ex: LoCo Public Schools) should not be used.

Comments:
An unannounced monitoring inspection was conducted today from 9:10am-11:50am. There were 40 children directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in sicence work "exploring cranberries" playing with blocks, using fine motor skills while learning to identify numbers, and practicing writing. A discussion was heald with the site director about using a currently unused space for a new classroom. Staff were observed to interact with children and other staff in a positve manner in a manner that promotes getting along. There was an variety of age appropriate supplies of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-280-B
Description: zardous materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A teacher's cabinet was observed with several spray bottles of cleaners, staff purses, and chemical sponges.
2. The cabinet was observed to be locked with the key inserted in the lock, rendering the lock ineffective.

Plan of Correction: We will remind the staff to put the key in a deesignated spece when not in use.

Standard #: 22VAC40-185-560-G
Description: Based on observation, when food was brought from home it ws not clearly dated and labeled in a way that identifies the owner.
Evidence:
Seven lunch bags and boxes were observed without a label indicating the date or the owner.

Plan of Correction: We will remind parents to label and date food from home nd ask that our staff be a back up in the event the parents do not provide the labels.

Standard #: 63.2(17)-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained.
Evidence:
One of the five staff records reviewed states that Staff 3 has lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff 3; DOH 08/2019).

Plan of Correction: A form to make the request was obtained during the inspection. We will request that the staff complete the form as soon as possible.
