Comments:
A Subsidy Health and Safety inspection was conducted on 11/18/19 from 10:50 am until 3:25 pm at Culpeper Baptist Child Development Center, Inc. to review supplemental health and safety requirements. At the time of the inspection 94 children were present with 23 staff present. The sample size consisted of nine children's records, nine staff's records, four diaper ointments and three children's medications and supporting documents. Children and staff were observed during indoor and outdoor free play, artistic and educational activities, story time, singing, preparing for lunch, hand washing, lunch, departure, transitions and behavioral guidance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-500-F
Description: Based on observation and interview, the vendor failed to ensure the current written list of all children's allergies, sensitivities, and dietary restrictions were accessible to all staff while also being kept confidential.

Evidence:

1. In the Llama classroom hanging on the outside of a cabinet was a list of children's allergies, sensitivities, and dietary restrictions.
2. The director stated the list is supposed to be covered. The staff present are not familiar with this class as they are covering for the regular teachers who are out and they must have reviewed the list and forgot to cover the list back up.

Plan of Correction: All staff will be reminded that the list of children's allergies, sensitivities, and dietary restrictions must always be kept confidential.

Standard #: 22VAC40-665-540-A
Description: Based on record review and interview, the center failed to ensure documentation stating new staff are determined to be free of communicable tuberculosis (TB) is completed within the last 30 calendar days of the date of employment and is submitted at the time of employment and prior to coming into contact with children for staff hired after 12/15/18.

Evidence:

1. The record for nine staff were reviewed. Staff 2's start date was 10/2/19. The documentation of TB is dated 2/18/19.
2. The director verified she was unaware of this new requirement.

Plan of Correction: In the future all new staff will be required to provide documentation of the TB screening which shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Staff 2 will be required to provide an updated TB screening.
