Comments:
An unannounced monitoring inspection was conducted on 11/20/19 from 10:10 am until 1:05 pm at ACAC Preschool at Adventure Central to review the licensing standards. At the time of the inspection 88 children were present with 15 staff. The sample size consisted of five staff's records, three medications and supporting documents and eight children's records. Children and staff were observed during free indoor and outdoor play, artistic and educational activities, story time, music class, transitions and behavioral management. A violation was found during this inspection and is documented on the violation notice. If you have questions or concerns contact the licensing inspector at (540) 848-4123 for further assistance.

Description: Based on observation, the center failed to ensure the diapering surface is used only for diapering or cleaning children.

Evidence:

1. In the 3's classroom, toys, papers and wipes were on top of the diapering surface.
2. The director stated all the children are potty trained but the changing table is used for accidents.

Plan of Correction: The teacher immediately removed the items from the diapering surface. All staff will be reminded to make sure the diapering surface is only used for diapering and items are not put on top of the surface.
