Comments:
1/28/2020 - The inspection report was amended as a result of a second step review.

An unannounced monitoring inspection was completed on November 15, 2019 from approximately 10:00 a.m. to 1:00 p.m. Thirty-two children were in care and nine staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, a sensory activity, group reading, outside play, lunch time, and transition to nap time. Interviews and interactions were conducted and medications were reviewed. Five children?s records and nine staff records were reviewed. The director and administrative assistant were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for two of nine staff.
Evidence: 1. Staff #2's record, hired on March 11, 2019 as a program leader, lacked documentation of the training required for the job position. Staff #8's record, hired on February 27, 2019 as a program leader, lacked documentation of the training required for the job position. 2. In an interview with Administrator #1 she reported that these staff have competed the required amount of training and are qualified program leaders but acknowledged that the training is not documented in these staff records.

Plan of Correction: The director will collect training documentation from the staff.

Standard #: 22VAC40-185-260-B
Description: Based on a review of records and interview, the licensee failed to provide licensing annual approval from the health department for water supply, sewage disposal system and food services if applicable.
Evidence: 1. Licensee presented a health inspection report dated October 29, 2018, and therefore not within the annual time frame. 2. In an interview, Administrator #1 acknowledged this inspection is overdue.

Plan of Correction: The owner will call the health department today to get the inspection scheduled.
