Comments:
An unannounced monitoring inspection and facility tour was conducted from 12:00pm-1:20pm. At the time of inspectors' arrival there were 28 infant to preschool aged children in care with 8 staff members. Children were observed participating in nap, handwashing and toileting. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medication were reviewed. The areas of noncompliance were discussed with the director during this inspection and are detailed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required information.
Evidence:
The record for child #3 did not contain the address for one of the two required emergency contacts.

Plan of Correction: Staff will request this information from parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. In the infant room there was a child in a crib who had a lanyard around his neck with his pacifier attached to it, presenting a potential strangulation hazard.
2. In the infant room there was fraying carpet at the seams in the center of the room, presenting a potential trip and/or choke hazard.
3. In the girls restroom utilized by children in care their was an open box of vinyl gloves on a shelf under the changing table, presenting a potential choke hazard.

Plan of Correction: The lanyard was removed and will be replaced with a clip on pacifier holder.
The frayed carpet will be trimmed.
The gloves were removed and will be kept out of the reach of children.
