Comments:
The licensing inspector conducted an unannounced renewal inspection on Monday, November 18, 2019 between 10:40 am to 1:50 pm to determine the provider's compliance with licensing standards at the child day center. During this inspection, there were 49 children and 11 staff present at the center including administrative staff. The licensing inspector completed observations of the children in care participating in a variety of activities to include engaging in active play with age appropriate equipment, sleeping in cribs, transitioning indoors from outdoor play, participating in a painting activity, dancing to interactive music, completing bathroom breaks, playing at outdoor playground area, and eating lunch. The children's lunch today consisted of turkey/ham and cheese, green beans, and pears, milk. The center's playground areas and designated children care equipment and areas were inspected. There were five children?s records and seven staff's record reviewed during this inspection.The first aid kit and emergency supplies, posted emergency contacts, emergency drills were reviewed by the inspector. Standard #: 22VAC40-185-140-A
Description: Based on review of children's record and interview with staff, the center did not ensure to obtain physical examination documentation within one month after attendance.

Evidence: In Child#2's record (start date: 3/15/19) and Child#3's record (start date: 6/17/19, there were no physical examinations found in the records. Interview with staff confirmed the documentation for physical examination for children were not available for review during the inspection.

Plan of Correction: The center will obtain all required information to include wellness checks, immunizations, and emergency contacts prior to enrollment.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's record and interview with staff, the center did not ensure to maintain the following information in each child enrolled record.

Evidence: In Child#1's record (start date:11/04/19) the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached was not available. In Child#2's record (start date: 03/15/19), the name, address, and phone number of one designated people to call in an emergency if a parent cannot be reached was not available.
Interview with staff confirmed the designated people information for children's record were not available for review.

Plan of Correction: The center will obtain all required information to include wellness checks, immunizations, and emergency contacts prior to enrollment.
