Provided training on updated Emergency preparedness plan, Proof of identity requirements and 3 hour required training to be completed by March 2020. - Vendor does not have MAT training and policy states she will not give emergency medication. No medications on site.

An unannounced SHSI inspection was conducted today from 9:50 am- 11:30 am. There were 4 children ( 9 points) in direct care of the vendor. Vendor information, household member information, child information and emergency preparedness planning and supplies were inspected. The children were observed: playing a matching game, drawing their family on the chalk board, watching a movie and playing with legos. Snack of oranges and milk was served during the movie. Hand washing routines and diaper routines were also discussed/ observed. If there are any questions regarding this inspection please contact kelly.savittiere@dss.virginia.gov

Standard #: 22VAC4-665-340-H Description: Based upon information reviewed, required information was not able to be determined. Evidence: The IKEA crib Model SUNDVIK had a manufacture date stamped 134851. Inspector unable to determine manufacture date at time of the inspection. Plan of Correction: I will call IKEA to get date.
Standard #: 22VAC40-665-160-B Description: Based upon information reviewed, The vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs . Evidence: There were 4 children in care and no times documenting their arrival into care today. Plan of Correction: I thought mommy had to sign she was in a hurry this morning. I will write time when they get here from now in.
Standard #: 22VAC40-665-270-A Description: Based upon observation, areas of the home were not free of hazards. Evidence: The leather sofa in the child play area had multiple large tears exposing foam inside. Plan of Correction: I will fix.
