Comments:
An unannounced monitoring inspection was completed on 11/22/19. There were 103 children present. Eleven children's records and nine staff records were reviewed. Two medications were reviewed. A self-reported incident was also reviewed during this inspection related to a bite to a child and ratios in the infant room.

The inspector arrived at the center at 7:20 am and departed at 1:15 pm.

The children were observed during arrivals, during breakfast, during classrooms times, during play land (inside gym) times, and during nap. All infant activities were observed.
The ratio in the morning hours for the infant classroom was observed.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview, the center failed to ensure that when cloth diapers are used that a separate leak-proof covered receptacle for soiled linens that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence:
1. An infant was observed being diapered. The staff person was asked about the use of cloth diapers. The staff person explained the system of "disposal" of the soiled diapers is in a zippered bag where the staff must use their hand to open the bag. The zippered bag with soiled diapers is stored in a cabinet on a shelf beside the child's basket of clean diapers.

2. The inspector observed the practice in Evidence #1.

Plan of Correction: The director will provide a separate step can for the cloth diapers that the parent will pick up daily.
