Technical Assistance:
Technical assistance was provided regarding requirements for individual contractors.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-1:30pm. There were 96 children (infants-5yrs.) supervised by 21 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, circle time, and building. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on review of five children's records, three who are under the age of two do not have documentation of updated immunization record every six months.
Evidence:
Children #1, 2 and 3 are under the age of two and do not have documentation of updated immunizations every two months, last update was 11/14/17, 5/29,18 and 1/2/19 respectively.

Plan of Correction: I will get the updated immunization records for all children under the age of two.

Standard #: 22VAC40-185-370-5
Description: Based on observation and review of documentation, 30 minutes of tummy time is not being recorded daily for Infants, who cannot turn themselves over.
Evidence:
There is no documentation of tummy time for infants who can not turn themselves over.

Plan of Correction: We will begin recording the 30 minutes daily today.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed after toileting.
Evidence:
A child did not wash their hands after toileting.

Plan of Correction: We will ensure all children's hands are washed after toileting.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of five staff records, one does not have documentation of an out of state background check from a state they have lived in within the past five years.
Evidence:
Staff #2 does not have documentation of an out of state background check from a state they have lived in within the past five years.

Plan of Correction: I submitted the form on 9/11/19, I called today to determine the status of the results.
