Comments:
An unannounced monitoring inspection was conducted on 11/21/19 from 10:50am until 12:50pm. At the time of entrance, there were 69 children in care with 13 staff present. A sample of 7 children's records and 7 staff records were reviewed. Children were observed engaging in morning learning activities, playing on the playground and eating lunch. Diapering, restroom and hand washing procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of administration and background checks are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years.

Evidence:
1. The record for child 1 does not contain documentation of an updated immunization record. Child 1, age 3 years, has a documented date of enrollment of 7/15/16. The last documented immunization record is dated 7/14/16.
2. The center director confirmed that the record for child 1 does not contain updated immunization records every six months before the child turned 2 years old, as required by the standards

Plan of Correction: The center responded with the following: Management will develop a tickler file on or before 12/15/19, to manage children?s immunization records to ensure compliance. AD will be responsible for maintaining and updating this file.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the center did not ensure that a central registry finding was obtained for all new employees within 30 days of employment.

Evidence:
1. The record for staff 1 does not contain documentation of a central registry finding. Staff 1 has a date of hire of 9/4/19 and was working on the day of the inspection.
2. The center director confirmed that the record for staff 1 does not contain documentation of a central registry finding.

Plan of Correction: The center responded with the following: Effective today, 11/22/19, management will calendar all criminal record requests when sent off, to ensure compliance or timely follow up within 30 days. DM will review for compliance quarterly. This particular record was emailed to the school from Richmond 11/23/19.
