A Subsidy Health and Safety Inspection was conducted on November 19, 2019 to review supplemental health and safety requirements. Licensing inspector arrived at 9:15 am and departed at 1:05pm to conduct a Subsidy Health and Safety Inspection. There were 68 children present upon arrival to the center with 12 staff members. Additional staff arrived throughout the course of the inspection who were on site for administrative and support purposes. There is a total enrollment of 68 children. The children were observed during center time in three classrooms and outside playing. Six children's records were reviewed. Eleven staff records were reviewed. The exit interview was conducted with the Program Director.
A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on record review, in one of five staff records reviewed, the vendor did not ensure that staff shall be ssued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
