Comments:
An unannounced monitoring inspection was conducted today 11/22/19, from 9:20am to 2:30pm. There were 47 children present with 13 staff. Children were observed singing and dancing with handkerchiefs, learning about foods, reading books, building with legos, being fed a bottle, napping, having diapers changed, hand-washing, and having tummy time. Reviewed 4 children's files, 4 staff files, emergency evacuation plan and drills, emergency supplies, first aid kit, attendance, menus, fire and health inspections, and physical plant. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's files did not have required information.
Evidence:
1. Child#1 did not have the address for 1 emergency contact.

Plan of Correction: We will get the information from the parent.

Standard #: 22VAC40-185-90--A
Description: Based on children's record review, a signed written agreement between the center and the parent was not obtained prior to the child's first day of attendance.
Evidence:
Child#2 (start date 9/18/19) and Child#3 (start date 9/25/19) did not have parent signatures on the Parent Information Statement.

Plan of Correction: We will obtain the signature from the parents

Standard #: 22VAC40-185-190-A
Description: Based on record review, Center Director does not have required qualifications for the position.
Evidence:
Staff#1 does not have required documentation for being Center Director.

Plan of Correction: We will immediately have someone who meets Director qualifications onsite starting Monday.

Standard #: 22VAC40-185-210-A
Description: Based on record review, staff who are being used as Program Leader do not have required documentation for the position.
Evidence:
Staff#2( hire date 10/23/2019) was observed alone in a classroom with 6 children, and was being utilized as Program Leader during this inspection, but did not have documentation of being qualified.

Plan of Correction: We will move staff to ensure there is a Program Leader qualified in the rooms.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment are not being maintained in a clean, safe and operable condition.
1. Observed missing flooring near the diaper changing station in Toddler A.
2. Observed peeling and chipping paint on the gate in Toddler A.
3. Observed soft play equipment in Toddler B with exposed stuffing and shredded threads.

Plan of Correction: Work orders have been placed to repair these items. The soft play equipment will be removed and a new on e ordered on Monday.

Standard #: 22VAC40-185-340-D
Description: Based on observation and record review, each grouping of children did not have at least one staff who is Program Leader qualified.
Evidence:
Staff#3 (start date 3/25/19) was observed being utilized as Program Leader but is not qualified for the position, This violation was previously cited on 6/6/19 for the same staff person.

Plan of Correction: We will move staff to ensure there is a Program Leader qualified in the rooms.
