Comments:
An unannounced mandated Monitoring inspection was conducted today. There were 55 children present with 11 staff. Children were observed building with legos, painting, having circle time, reading books, building with colored shaped, and practicing phonics. Reviewed 5 staff files, 5 children's files, medications and authorizations, attendance, and physical plant. Violations of the standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on children's record review, each child did not have the required information in the files.
Evidence:
Child#2 (last update 7/24/18) and Child#5 (last update 8/26/18) did not have documentation of an annual update of child's information.

Plan of Correction: We will obtain the information and signature from the parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment in the center are not being maintained in a safe, clean and operable condition.
Evidence:
1. Observed dirt and dust covered vents throughout the center.
2. Observed floor molding in the Preschool B room to be cracked and jagged,
3. Observed the plastic cover of a light fixture to be missing in the Preschool B bathroom,leaving exposed glass bulbs.

Plan of Correction: A work order has been placed for the vents and work orders will be placed for the other items.

Standard #: 22VAC40-185-340-A
Description: Based on interview, staff were unaware of how many children were in their care when asked.
Evidence:
Staff#1 was unaware of how many children were in there care. Staff#1 had to ask another staff member how many children they had in the room. This violation was previously cited on 5/31/19 regarding the same staff person.

Plan of Correction: I will re-coach the teachers on knowing how many children they have in their care at all times.

Standard #: 22VAC40-185-440-B
Description: Based on observation, cots were not identified for use by a specific child.
Evidence:
Cots throughout the facility were not labeled for a specific child.

Plan of Correction: We will make sure all cots are labeled to identify the child assigned to use it.
