Comments:
An unannounced monitoring inspection was conducted on 11/20/19 from 2:51 pm until 5:30 pm at ACAC Afterschool Kidz Club. At the time of the inspection 217 children were present with 14 staff. The sample size consisted of five staff's records, 10 children's records and five children's medications and supporting documents. Children were observed during free and organized indoor and outdoor play, during artistic activities, watching a movie, pick-up, transitions and behavioral guidance. A violation was found during this inspection and is documented on the violation notice.

Standard #: 22VAC40-185-510-E
Description: Based on observation and interview, the center failed to ensure a medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence:

1. An Epipen was found in a medication bag for child M4. The Epipen was not in the original box with the prescription label and did not contain any other label.
2. The administrator was unable to produce the original box or label.

Plan of Correction: The Epipen will be labeled and the parents will be advised to provide a prescription label.
