Comments:
A Subsidy Health and Safety inspection was conducted on 11/20/2019 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from 10:45am-3:15pm. There were 55 children (infants-4yrs.) directly supervised by 12 staff. The physical plant, 6 staff records, 6 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, playing with blocks, drinking milk, napping, being bottle fed, playing with legos, cutting patterns, playing outside and eating lunch. Today's lunch consisted of black beans, whole grain spaghetti, peaches, mixed fruit and milk. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on record review, the center did not obtain a statement that the individual is determined to be free of communicable tuberculosis (TB) prior to coming into contact with children.

Evidence:
Staff#6's record contains a TB screening/test that was signed by the physician and dated 7/31/2019. The staff member's first day of employment was documented as 6/20/2019.

Plan of Correction: We will ensure TB screenings are done prior to staff's first day of employment.
