Technical assistance was provided today regarding: background checks, outdoor play area, medications, potential change in ownership, staff files and staff qualifications.
- Comments:

An unannounced monitoring inspection was conducted today from 10:00am-1:00pm. There were 24 children directly supervised by 4 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. Lunch served today was rice, beef strew, tofu with vegetables and apples. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-140-A Description: Based on record review, each child did not have a physical examination by or under the direction of a physician.
Evidence: Child 1 does not have documentation of a completed physical examination by or under the direction of a physician.Plan of Correction: We will contact parents to provide us with the physical examination records.
Standard #: 22VAC40-185-160-C Description: Based on record review, a staff member did not submit a follow-up tuberculosis screening at least every two years from the date of the most recent screening.
Evidence: Staff 3 has a tuberculosis screening which was last conducted on 7/17/2016.Plan of Correction: We will ask the staff member to complete a new tb screening.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff do not have written information to demonstrate that the individual possesses the education or certification required by the job position.
Evidence: Staff 1 does not have documentation to support their qualification for their position.Plan of Correction: We will find out an evaluation organization so the documentation can be verified.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure all staff have a completed central registry finding within 30 days of employment.
