63.2 Facilities & Programs.

Comments:
An unannounced monitoring inspection was conducted today from 3:00pm-4:30pm. There were 63 children directly supervised by 6 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed checking into the program, reading books, socializing with peers, playing games and interacting with staff. Staff interactions with children were observed and deemed positive. Children were greeted as they entered the program, talked to calmly and were reminded to wash hands before snack.
Snack options served today were pancakes, cereal, cheese crackers and juice, water, or milk. There was a supply of materials for the children. One area of non-compliance is identified in the Violation Notice.

Description: Based on record review, the center did not ensure all information with the file.

Evidence: Child 1 is missing addresses for both emergency contacts.

Plan of Correction: We will continue to follow up with the parents regarding the addresses.
