Comments:
An unannounced monitoring inspection was conducted in the center on 11/21/2019. The inspector arrived at 8:50 A.M. and departed at 11:30 A.M. There were 46 children observed during the inspection. The children were grouped in 4 classrooms. The ages of the children ranged from infancy to preschool. The center exceeded staff to child ratios in each group. The children were observed during outdoor play. Staff was observed with the infants outdoors for a ride in the stroller. The children were observed during art activity, bubbles and at the water table. individually selected centers. Diapering and hand washing procedures were observed.
A sample of 5 children's records was reviewed. Complete staff records are maintained off site and were reviewed earlier. Documentation of the annual fire and health inspections was reviewed; Medications were observed in the center.

Description: Based on review of the injury plan, the center failed to ensure that the plan was updated annually.

Evidence:
1. The injury plan was updated August 2018.

Plan of Correction: The main office will work on the injury plan to ensure that the update is completed annually.
