An unannounced SHSI inspection was conducted today from 12:50 pm- 2:10 pm. There were 2 children ( 6 points) in care at the time of the inspection. Provider/ household member information/ child information was reviewed. The children were observed: napping, building with blocks and doing tummy time. Hand washing and potty routines were observed and discussed. If there are any questions regarding this inspection please contact kelly.savittiere@dss.virginia.gov

Standard #: 22VAC40-665-170-B Description: Based upon information reviewed, required child information was incomplete. Evidence: Child A's information is missing emergency contact information( name/address/telephone number. Child B's information is missing emergency contact information ( name/address/telephone number). Plan of Correction: I will ask parents for information.
Standard #: 22VAC40-665-210-B Description: Based upon record review, required immunization records were not up to date. Evidence: Child B (DOB 10/4/16) was not current. Last record on file was 11/2017. Plan of Correction: I will ask mom for updated immunization record when she comes back after the summer.
Standard #: 22VAC40-665-410-B Description: Based upon records reviewed, vendor did not maintain record of the dates of practice drills as required. Evidence: There was no documentation of a fire drill being completed in for the month of May 2019. Plan of Correction: I will make sure to document every month.
