An unannounced monitoring inspection was conducted beginning at 10:30 a.m. and ending at 1:30 p.m. with the center director. There were 32 children in care ranging in age from 5 months through 5 years old under the care and supervision of 6 staff. Infants were observed having play time on the floor and during diapering. Toddlers were observed during play time. Two year olds were observed playing with play doh and playing with different toyf vehicles. Preschoolers were observed at various centers that included sensory table, imagination play and playing with balls. The PreK class was observed during an art project about healthy bodies. The outdoor play areas and playground were not inspected today due to the weather.
Seven children's records and four staff records were reviewed. There are no medications on site. Emergency drills were reviewed and up to date.

Standard #: 22VAC40-185-130-B Description: Based on review of documentation, it was determined that the center did not obtain documentation of immunizations once every six months for children under the age of two years. Evidence: Child #2, date of birth 6/28/18, last documented immunization was dated 9/17/18. Plan of Correction: This information will be requested from the parents as soon as possible.
Standard #: 22VAC40-185-140-A Description: Based on review of documentation, it was determined that the center did not obtain documentation of a physical within 30 days of enrollment. Evidence: Child #2, enrolled 10/17/18, did not have documentation of a physical in the record. Plan of Correction: This information will be requested from the parents as soon as possible.
Standard #: 22VAC40-185-60-A Description: Based on review of documentation, it was determined that the center did not obtain documentation of required information for children's records. Evidence: Child #1, enrolled 6/519, ddi not includ father's information, name and phone number of employer, an address for one emergency contact and a phone number for a second emergency contact. Child #2, enrolled 10/17/18, did not have an address for a second emergency contact. Child #5, enrolled 11/20/18, did not have father's employer and phone number and an address for a second emergency contact. Child #7, enrolled 11/8/19, did not have a second emergency contact and father's employer and phone number. Plan of Correction: The information will be requested from the parents as soon as possible.
