Technical Assistance:
Posting of inspection notices was discussed with the program director.
Also discussed was the storage of janitorial or cafeteria chemicals when children are in care.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 7:30 AM and departed at 8:20 AM. Morning arrivals and activities were observed. Medication administration was reviewed. No children's injuries were reported. A sample size of five children's records were reviewed.

A review of the acknowledgement form was conducted with the program director prior to the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes the names. street addresses and telephone numbers of two designated persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
1. The record provided for one of five records reviewed did not include complete street addresses for the emergency contact persons listed.
2. Staff 1 verified that the addresses indicated only the cities and states and did not include street addresses.

Plan of Correction: The facility responded with the following:
All chlldren's records will be checked to ensure that complete street addresses are included for the designated emergency contact persons.

Standard #: 22VAC40-185-510-N
Description: Based upon review of medication stored at the facility and staff interview, the facility has not ensured that when medication expires, the parent ios notified that the medication must be picked up within 14 days of the facility will dispose of the medication.
Evidence:
1. There was a ventolin inhaler stored in the medication box that expired in June 2019 that was not returned to the parent or disposed.
2. Staff 1 verified that the medication was expired and should have been returned to the parent.

Plan of Correction: The facility responded with the following:
Any medication that is expired or for which the parent authorization has been expired will be returned to the parent or, if not picked up, will be disposed.
