Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 7:05 AM and departed at 8:10 AM. Morning arrivals and activities were observed. A sample size of five children's records were reviewed. It was reported that there currently is no medication at the site for administration to the children. No children's injuries were reported.

The acknowledgement form was reviewed with the program director prior to the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon review of children's records and staff interview, the facility has not ensured that each record contains the names, addresses and contact numbers for two designated persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
1. The record reviewed for child 2 did not include a street address for one of the two designated emergency contact persons.
2. The record reviewed for child 5 included only one designated emergency contact person in addition to the parents.
3. Staff 1 verified that additional emergency contact information is needed in both files.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that they contain the names, address and telephone numbers for two designated persons other than parents.

Standard #: 22VAC40-185-350-E-4
Description: Based upon observation and staff interview, the facility has not ensured that for children from three years to the age eligibility to attend public school, there is one staff member for every ten (10) children.
Evidence:
1. At 7:40 AM, there were twenty-three (23) children ages four years through twelve years in care with two staff.
2. Staff 1 verified that there were twenty-three children and that the youngest children were pre-K (four years old).

Plan of Correction: The facility responded with the following:
Additional staff will be hired for the site and children will be in separate groups according to age and required ratio.

Standard #: 22VAC40-185-540-E
Description: Based upon observation and staff interview, the facility has not ensured that there is a working battery operated radio.
Evidence:
1. There were no batteries available for placing into either radio at the site.
2. Staff 1 verified that there were no working batteries for the radios.

Plan of Correction: The facility responded with the following:
AA and C batteries will be purchased and maintained in the emergency supply bin.
