Technical Assistance:
Technical assistance was provided regarding sending the inspector the CPS results when they are received. It is suggested that CPS forms should be sent in ASAP after hiring to ensure results are received back within 30 days.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-12:30pm. There were 65 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 5 staff records, 5 children?s records, 10 medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and making cookies. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of five staff records, one does not have documentation of a negative tuberculosis (TB) screening within 21 days of hire.
Evidence:
Staff #1, date of hire 9/1/19, does not have documentation of a TB screening within 21 days of hire.

Plan of Correction: Staff will get the TB screening done by next week. I will make sure staff have it completed within 21 days of hire.

Standard #: 22VAC40-185-70-A
Description: Based on review of 5 children's records, one is incomplete.
Evidence:
Child #1 does not have documentation of addresses for two emergency contacts.

Plan of Correction: I will contact the parent and get the information, in the future we will get the information before the child's first day of attendance.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, one does not have documentation of a central registry finding within 30 days of hire.
Evidence:
Staff #1, date of hire 9/1/19 does not have documentation of central registry finding within 30 days of hire.

Plan of Correction: I mailed the form in a week ago, I will email the inspector the results when they are received.
