12:45 pm- 2:40 pm. There were 34 children supervised by 5 direct care staff. The physical plant, 3 staff records, 5 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing were inspected. The children were observed in free play and in center activities. Hand-washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance. An unannounced monitoring inspection was conducted today approximately

Standard #: 22VAC40-185-60-A Description: Based on records, 3 out of 5 children records, the center did not maintain and keep at the center a separate record for each child enrolled the which shall contain the following information:
Evidence:
1. Child #2's record was missing an address for one designated person to call in an emergency if a parent cannot be reached and previous child day care and schools attended.
2. Child #3's record was missing address? for two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: We have requested all the missing information from parents. All the missing date has been submitted, all children's records have been updated
Standard #: 22VAC40-185-90--A Description: Based on records, 5 out 5 children records, the center did not have written parental agreement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Evidence:
1. Child #1, #2, #3, #4, and #5 did not have written parental agreement of reportable communicable disease.Plan of Correction: We have requested and received written parental agreements that the parents will inform the center within 24 hours at any reportable communicable disease.
