An unannounced monitoring inspection was conducted today, November 1, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 1:10 pm and concluded at 4:00 pm. Upon the inspector's arrival most children were napping. Two children were sitting at the table playing with a tablet and having a snack. After nap time, children had bathroom breaks and washed their hands before sitting down to snack. Children also were observed during center time where they played with the doll house, kitchen set and cars. A small group of children painted pictures and later children danced to a music video on the smart board. All areas of the center were inspected. The center has age appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. The center has a parent board with the required documentation posted. The Inspector interacted with staff and children in care.
Five children's records were reviewed along with three staff records.
Per the director, medication is currently not being administered.
The most recent evacuation drill was documented on 10/17/2019. A shelter-in-place drill was conducted on 03/20/2019. A second shelter-in-place drill will need to be completed by the end of the year. A lock down drill was completed on 08/26/2019.
Today's census:
Infants and toddlers combined: 5 children with 3 staff.
two-five years: 15 children with 2 staff.
school-age: 2 children with 1 staff.
Today's lunch consisted of fish sticks, green peas, mashed potatoes, wheat bread and milk.
Violations cited during the licensing period were checked for corrections. There were no repeat violations cited during today's inspection.

Standard #: 22VAC40-185-130-B Description: Based on review of children's records, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
The most recent documentation of immunizations for child #5, 19 months in age, was dated 05/16/2018, exceeding the requirement to obtain updated immunizations every six months for children under two years.Plan of Correction: Per the director: I will contact the parent to get a copy of the updated immunization record for the file.
Standard #: 22VAC40-185-140-A Description: Based on review of children's records, the center did not obtain documentation of a physical examination before the child's attendance or within one month after attendance.
Evidence:
1. The record of child #1, enrolled 02/04/2019, did not have documentation of a physical examination, exceeding one month from enrollment.
2. The director confirmed there was no documentation of a physical examination in the record for Child #1.Plan of Correction: Per the director: I will contact the parent and have them provide documentation of a physical for the record.
Standard #: 22VAC40-185-420-E-1 Description: Based on interview, the center did not maintain a posted daily record which can be easily accessed by both the parent and the staff working with the child that contained all required information.
Evidence:
1. Infant #1 and infant #2 did not have a daily record with all required information listed.
2. Staff stated they had not documented any of the required information for November 14, 2019.
3. Staff stated they did not document tummy time as required.Plan of Correction: Per the director: We will add tummy time to our infant daily reports and make sure staff document all required information as it occurs.
Standard #: 22VAC40-185-560-N Description: Based on observation, the center did not ensure children were not permitted to eat or drink while walking.
Evidence:
1. A child was observed walking in the play area with food.
2. Staff acknowledged the child was eating while walking around the classroom.Plan of Correction: Per the director: We will have the children stay at the table while eating.
