Comments:
An unannounced monitoring inspection was conducted on Monday, November 18, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 10:25am and concluded the inspection at approximately 1:25pm. The center?s administrator and the center?s director were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 57 children in the direct care of 12 staff members. Additional staff were on site and available to the children in care. Upon arrival, the children were engaged in a variety of activities. The children in the Younger Toddler class were observed outside in their designated playground. The children in the Lower Nursery were observed as they entered their classroom from the playground. The children washed their hands, took bathroom breaks, and sat down for a story. The staff were observed assisting the children and interacting with the children in a positive manner. The children in the Upper Nursery were observed washing hands and listening to a story while they waited for lunch to be served. The children in the Preschool class were observed in different centers during their free-time period. Some were observed completing two different art activities related to the theme of the week. The children in the Pre-K class were observed completing an art activity with a staff member while others played in different centers and interacted with another staff member and peers. The children were later observed lining up to go outside. The children in the Jr. Kindergarten class were observed in different centers, completing an art activity, and playing on the carpet as a group. The children in the half-day Preschool program were observed in the playground and later in their respective classroom listening to a story. There were no school-age children present today. The center is equipped with age and stage appropriate materials and equipment for the children?s use. Staff were engaged with the children and were observed offering guidance when needed. The areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. Medication is administered and five emergency medications were reviewed today. During the inspection, five children?s and five staff records were reviewed. The most recent evacuation drill was documented on 11/08/2019. The most recent shelter-in-place drills were documented on 03/22/2019 and 08/22/2019.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records and interview on 11/18/2019, the center did not ensure one child had a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance.

Evidence: 1) Child #5 (date of enrollment 09/04/2018) did not have a physical examination on file on the date of the inspection. 2) During interview, a member of management reported there was no physical examination on file for Child #5.

Plan of Correction: Per the Center: The parent for Child #5 has been contacted and a copy of the physical examination has been requested. Administration will follow up in one week. In the future, the administration will make sure physical examinations are submitted within the required time frame.

Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records on 11/18/2019, the center did not ensure one staff submit documentation of a negative tuberculosis (TB) screening within the required time frame.

Evidence: 1) The TB results in the record for Staff #1, hired on 09/06/2019, were dated 10/02/2019. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Plan of Correction: Per the Center: TB results for Staff #1 have already been received. In the future, resources other than the Health Department will be used to complete TB screenings and testing.
